Description Of Item: 297 mm W x 150 mm H cream paper, printed one side and headed 'Application form for membership' and the VOA crest. The form was to be returned to HC Spencer, the honorary secretary at Kurrajong House, 177 Collins Street. The fee for membership was one pound 11 shillings and sixpence (aprox $3.15)
